Please complete as much of this form as possible - as it provides us the basic information about the individual we are providing support to.

This forms provides you information about our service provision, including confidentiality and cancellation policies.

This form will provide permission for us to consult with any other treating providers or services involved in the individual's care team.

PP: RAHC Authority Form (Adult Clients)

This form is compulsory for adult individuals with a legal guardians or representatives, to ensure that we have appropriate consents.

PP: Consent for Student Client Allocation

Please complete this form if our student or therapy assistant has been allocated to your child care team.

Complete this form if your child is undergoing an Autism Assessment which will involve video recording of the ADOS-2

C. Baseline Assessments for Adults

These forms are required for those seeking to engage in a diagnostic assessment at Ripples.

To maximise the effectiveness of your in-person consultation and streamline the process, we recommend thoroughly reviewing the online questions and preparing examples in advance. This will help facilitate on-the-spot discussions about historical examples during the interviews. Please be aware that if additional time is required during the appointment, any excess time will incur additional charges beyond the originally agreed-upon quote.
